If you are sending us equipment for repair via UPS, FedEx, USPS, or freight, use this form.

We require an $107 deposit / evaluation fee per piece of equipment that gets repair. For your safety we will not ask you to type in your credit card information in an information box. After receiving the product into our shop, we will contact you for credit card information via the phone number or via the e-mail address that you provided.

Additionally, you may also send the deposit / evaluation fee via check with the shipment, or send through Paypal.
